A/C crashed in a soybean field and burned at approx 1900 hrs 12 mi south of Clarksville off Gholson Rd during a routine night training flight. At approx 1848 hrs, flying 120 KIAS and 700' AGL, A/C began to pitch up and roll left, followed shortly by M/R pitch housing and M/R blade separation from rotor head, T/B separation from fuselage and remaining M/R system separation from xmsn. Crash was caused by P/C link rod end bearing not being staked onto the #3 lower rod end.

A/C crashed and burned on a mountain road in Wonju in Gangwon Province at approx 1820 hrs. A/C conducting APART flight eval departing at 1656 hrs, flew to a maintenance test flight area and conducted combat maneuvering flight. A/C then flew to a training area where they conducted terrain flight maneuvers. During evaluation, the weather began to deteriorate, and A/C departed at 1818 hrs enroute to subsequent training area. While transitioning to the next training area, A/C impacted a large set of power lines at 395' AGL and impacted with the ground causing a post-crash fire.

A/C crashed at approx 1900 hrs south of Gowen Field while on a routine annual proficiency evaluation training mission. While performing a simulated NP low fail condition at 435' AGL, #2 engine was inadvertently taken out of the fly detent while #1 engine was placed in lockout position, causing both engines to accelerate to the overspeed protection trip point which resulted in a dual-engine shutdown, rotor RPM decayed and A/C impacted the ground.

A/C crashed at approx 0900 hrs in Gera Khel area of Pachir Aw Agam district of Nangarhar province while conducting recons of various checkpoints in support of ground forces. A/C was #2 in flight of 2 and was flying at 94 KTAS and approx 2000' AGL providing high security over watch for lead when radio contact was lost. Lead A/C observed fire on the ground where trail acft crashed. Cause is suspected to be a material failure in the M/R system which caused a loss of control and subsequent crash.

A/C crashed in the Nika district of Paktika Province. A/C was in flight of 2 that departed FOB in support of troops in contact. A/C split up to cover the NAIs and entered a restrictive wadi in rising mountainous terrain at approx 75 KTAS and 300-500' AGL. Due to rising terrain, a steep left-hand turn exceeding 50 deg of bank was executed while at 34 KTAS and 70' AGL. During the turn, rotor RPM drooped and crew recovered by establishing a quick rate of descent, however prior to impact, rotor slowed below 86% causing generators to drop offline and A/C impacted ground, tearing apart the underside, then impacted a steep rock wall nose first and came to rest on its right side.

A/C crashed in Tarin Kowt after an engine #1 oil PSI caution and pilot elected to pull the power lever to idle. Conditions where A/C was operating did not allow for adequate single engine flight. Pilot drooped the rotor, yawed, impacted the ground and an intense post crash fire resulted.

A/C was conducting Combat Air Patrol in trail position when it came under heavy machine gun fire. Pilots called in to lead and announced they had lost utility hydraulics and could no longer fire gun, but could go fixed rockets. In spite of their severely damaged A/C, they radioed that they were ready to engage. They continued to provide cover while attempting to acquire and suppress the enemy to keep attention away from their wingman, when A/C was brought down near an island in the Tigris River near Taji.

A/C crashed in Western Baghdad after mid-air with another AH-64. A/C was lead in flight of two when wing A/C lost contact. Trail initiated lost contact procedures and moved back into position off lead's right wing. Lead called for a right turn flying relatively straight along a canal at approx 450' AGL in a slight climb. Trail was in a slight decent attempting to regain position when rate of closure was too fast and flew into path of lead, striking the M/R system of lead. Trail initiated emergency roll-on landing.

Crash occurred approx 20 min prior to sunset in light rain into a setting sun with a broken to overcast sky when pilots failed to detect wires suspended across the river 50' AGL, striking multiple 1.25 in. wires and crashed into the river.

On 3rd and final run, A/C approached at a faster rate than previous runs. During egress turn away from target, A/C entered a 15 deg. dive at 2,500 FPM rate of descent, impacting the ground at 134 KIAS in a nose-down, 26-deg. right bank.

A/C crashed into the dense thicket of trees and woods 5 mi W of Richmond Hill in a training area on Ft Stewart during a night practice mission. Crew mistakenly identified a 2nd aircraft from the one they were to follow, became disoriented and failed to notice their A/C was descending. A/C struck treetops approx 75' AGL which tore off the TADS, PNVS, left missile and rail, and part of the left wing. PC increased power and began a cyclic climb with power above max power available and rotor RPM continued to decay. At 1000' the A/C began to spin to the right and descend. At 200' the electrical power was lost due to low rotor RPM and A/C crashed.

A/C had just accepted the target handover for their next engagement when their radio abruptly went silent. When they failed to report "engagement complete". A/C had impacted the ground, causing the fire at approx. 2000 hours. The ensuing investigation into the cause of the accident revealed that there was no damage or failure prior to impact; the cause of the crash on the Barry M. Goldwater Range has never been determined.
